The appointment request form asks for: the service requested, preferred date and time, your name, mobile number, preferred contact method and consent. You may optionally provide an email address and a short message. The contact form asks for your name, a phone number or email address, an enquiry category and a message.
Please do not enter identity numbers, payment details or detailed medical history in these forms. We do not ask for them.
3. Purposes of collection
Information provided will be used to respond to your request or enquiry, to arrange or confirm appointments and to contact you in the way you prefer.
4. Contact and appointment enquiries
Appointment dates and times you choose are preferences only. The practice will contact you to confirm availability. Messages sent by telephone, WhatsApp or email are handled by those services under their own terms.
5. Information access and handling
Access to enquiry information should be limited to practice staff who need it to respond. The practice must confirm and document its own internal procedures before launch.

This website loads fonts from Google Fonts and styling and icon files from the jsDelivr network, which means your browser contacts those providers when you visit. The site does not set cookies or use browser storage for personal information.
8. Security considerations
No information is stored or transmitted by the current demonstration, so no security claims are made about it. Before any form collects real patient information it must be connected to a suitable backend and reviewed for security and privacy, including secure transmission and storage.
